PixelMatrixDesign has put together a wonderfully simple and clean jQuery script that will make your forms look absolutely stunning.  It’s called Uniform. Check it out at http://pixelmatrixdesign.com/uniform/.

Uniform masks your standard form controls with custom themed controls. It works in sync with your real form elements to ensure accessibility and compatibility.

At the moment there are two included styles available.  You can also create your own if you choose, but Pixel Matrix Design has done an amazing job already on the first two.
